dc.description.abstract | El daño cerebral perinatal (DCP) se produce en el periodo entre la semana de gestación 24 y los 28 días de vida extrauterina; altera principalmente la estructura del tejido nervioso y supone la afectación de diferentes funciones como las sensoriales, motrices y cognitivas. En los recién nacidos prematuros, la patología más común son las lesiones de sustancia blanca, que pueden desarrollar diferentes secuelas como las que se identifican en la edad escolar (déficit de atención, problemas de aprendizaje, etc.), las que generalmente representan un déficit funcional (parálisis cerebral, déficits motores, retraso mental, secuelas sensoriales, etc.) y los problemas de lenguaje Objetivo. Comparar el desempeño psicomotor de dos grupos de prematuros con DCP que fueron tratados con conceptos terapéuticos diferentes (Bobath y Katona) durante los primeros dos años de edad corregida. Método. Se incluyeron 36 pacientes: control sano (n=10), Bobath (n=12) y Katona (n=14). El tratamiento se llevó a cabo de manera intensiva tres veces al día hasta que el niño consolidó la marcha. Las valoraciones del desarrollo psicomotor fueron realizadas al inicio del tratamiento, a los 4, 8, 12, 18 y 24 meses de edad corregida con la escala de desarrollo infantil Bayley segunda edición, la consolidación de los hitos de desarrollo motor grueso, la escala de desarrollo psicomotriz y el desarrollo del cuerpo calloso (CC). Resultados. Durante el primer año el grupo con tratamiento Katona mostró mejor desempeño que el grupo Bobath, así como también la consolidación de los dos primeros hitos de desarrollo, sin embargo posteriormente el desempeño psicomotor fue similar. En el desarrollo del CC, no existe diferencia significativa en ninguna evaluación. Conclusión. La estimulación temprana planeada para minimizar secuelas del daño cerebral perinatal de aparición temprana mejoró el desempeño psicomotor. A pesar de las diferencias observadas durante el primer año, en el seguimiento a los dos años de edad corregida se observó que el desempeño conductual es normal cuando el tratamiento inicia en las primeras semanas postnatales. | |
dc.description.abstract | Perinatal brain damage (PBD) is a lesion that occurs in a period of time between the 24th gestation week and 28 days of extra uterine life; it mainly alters the structure of the nervous tissue and involves damage of different functions like the sensory, motor and cognitive. In the preterm newborn population, the most common pathologies are found to be white matter lesions that can trigger several outcomes with a different degree of severity like those that are identified at school age (attention deficit, learning problems among others) as well as those which are generally functional deficit (infant cerebral palsy, motor behavior disorders, mental retardation or sensory disturbances) and language disorders. Objective: To compare the psychomotor performance of two groups of premature infants with perinatal cerebral damage who were treated with different therapeutic concepts (Katona and Bobath) during their first two years of corrected age. Methods: 36 infants were included: healthy control (n=10), Bobath (n=12) and Katona (n=14). The treatment was performed intensively three times a day until the children walk independently. The evaluations were conducted with the BSID-II and a psychomotor development scale at 4, 8, 12, 18 and 24 month of corrected age, besides the achievement of gross motor development milestones (GMDM) and the measure of corpus callosum. Results: During the first year, the Katona group performed better than the Bobath, as well as the first two GMDM were achieved earlier for the Katona group, but later the psychomotor performance was very similar. There was no significant difference in the corpus callosum development. Conclusion: The planned stimulation to minimize the abnormalities that were initially seen improved the psychomotor performance. However, despite the difference between the first months of treatment, at the follow up evaluation at the age of two years corrected age, behavioral development is effective when treatment begins during the first postnatal weeks. | en |
